Centennial Medical Group, a division of CHI Mercy Health, is a multispecialty physician group with offices in the Roseburg area. We are one of the largest physician group practices in Douglas County and offer specialty care for all ages. Our professional team of providers includes specialists in orthopedics, cardiology, podiatry, general surgery, ophthalmology, gastroenterology, pulmonology, sleep medicine, nephrology, and pathology. CMG includes a diverse group of highly skilled providers so that you can find the provider or specialist you need right in your own community.

EpicCare Link

EpicCare Link is Epic’s web-based application for connecting organizations to their community affiliates. It provides community users secure access to select patient information in CHI Health’s Epic data repository.

EpicCare Link is not an EMR solution. It is a read-only application with some service-oriented features which are configured to facilitate patient care with community practices.

Examples of community users include people outside of CHI Health who might need to review the clinical and administrative information of patients seen at our organization.  Affiliate clinical staff, billers, long-term care facilities, and correctional facilities are examples of EpicCare Link users.

Step 1: Enroll Practice/payer

The Practice/Payer must be enrolled first. Use the enroll Practice/Payer link.

Step 2: Site Administrators

After the Practice/Payer is enrolled, site administrators must be designated for the Practice/Payer. Use the Site Administrator Request link.

The request link has two actions:

  • Requested Site Administrator completes their portion and forwards the request link to the Practice/Payer contact. This cannot be the same person as the site administrator.
  • The Practice/Payer contact completes and submits the request.

NOTE: Two site administrators per Practice/Payer are recommended.

Step 3: Individual Users

Once the site administrator has been enrolled, individual users can request access using the Request Individual User Access link.

The request has two actions:

  • End user completes their portion and saves the form. This will populate the site administrator’s email address into a link. The end user clicks to send the email.
  • The site administrator completes and submits the request.

Throughout the process, the requestor will be notified if the request is approved or denied. If you have any questions about the process or status of a request, please email RoseburgECLink@commonspirit.org.

Each EpicCare Link user must be associated with an enrolled health care organization and must still complete an Individual User Request.

Site Administrator Request

Request enrollment as a site administrator for a health care organization. Site administrators are responsible for resetting user passwords and validating user access every 90 days.
